Extending Your Care Into the Home

Beth’s Pawfect Pet Services is operated by a qualified, RCVS-registered Veterinary Nurse with clinical experience in post-operative care, medication management, wound care, and chronic disease support. Based in Telford, I work across Shropshire to provide in-home care for your patients, keeping the standard of care high and the stress on your patients low.

This page has everything you need: a downloadable referral form for patients to be referred to me, and an information sheet for your practice team explaining how I work and what to expect.

Services I provide through veterinary referral include for example:

  • Administer prescribed monthly injections (Librela, Cytopoint, Allergy injections)
  • Administering prescribed medicated ear or eye drops as part of a treatment plan
  • Post-operative care, such as wound management and stuture removal

Download this form and bring it to your vet. Ask your vet to complete and sign it before returning to Beth, either handing it to you or emailing it directly.

Your vet is welcome to email the completed referral form directly to beth@bethspawfectpetservices.co.uk or if you’d like me to send a blank referral form directly to your veterinary practice, just get in touch and I will send one over.

How the Referral Process Works

The process is designed to be simple for both the pet owner and the referring practice, with clear communication at every stage.

  1. Your vet completes the referral form
    You’ve vet fills in your pet’s details, the services required, and any specific clinical instructions, then signs and dates the form.
  2. Send the form to Beth
    Hand the completed form to me at your first appointment, or email it directly to beth@bethspawfectpetservices.co.uk
  3. Care begins with full records kept
    I will follow the vet’s instructions and keep a written record of every visit. If any concerns arise, your vet is contacted.
  4. Ongoing vet communication
    Visit records are available to your veterinary practice (and yourself) at any time on request. Any change to the care plan must come from the referring vet. I act as an extension of the veterinary team, not a replacement for it.

My Commitment to Referring Practices

Veterinary practices that refer clients to me can expect the following standards at all times:

  • Clinical standards – all care is carried out to the standard expected of an RCVS-registered Veterinary Nurse, following your clinical instructions. I do not diagnose or alter treatment plans.
  • Full record keeping – detailed visit records are kept after every appointment, including observations, medication given, and any changes noted. These are available to you on request.
  • Immediate escalation – if any concerns arise during a visit, I will contact the referring practice immediately. Emergency protocols are in place for every patient.
  • Confidentiality and GDPR – all client and patient data is handled in accordance with GDPR and treated with the same confidentiality expected in a clinical setting.
  • Fully insured and DBS checked – I hold insurance and I am DBS checked. Copies are insurance certificated and RCVS registrations are available to practices on request.
